How to Qualify for Trade Pricing:
To apply for trade pricing, please complete the application to follow and fax these supporting documents to our office at 406-582-8444:

1. Current resale license, business license and/or tax ID registration. The document must show the purchaser's name and address. This must match the name and billing address for all orders.

2. Copy of your Professional Association Membership information.
